Co-developing the Singapore Early Psychosis Intervention’s Inpatient Service

Researchers studied the impact of ward design and facilities on clinical and behavioural outcomes in patients. Service users have been engaged in the design process but few studies detail how they enhance care models. To share how service users co-develop Singapore’s early psychosis inpatient service. The Early Psychosis Intervention Programme (EPIP) was initiated in 2001 funded by Singapore’s Ministry of Health. Holistic and comprehensive care is provided by inter-professional team of caregivers and persons in recovery and healthcare professionals. In 2015, EPIP received funding for a specialized inpatient ward. EPIP participated in lean workshops to identify and reduce inefficiencies, and ensure safety. Focus group with service users was conducted and the team learned best practices. During brainstorming, direct admissions was suggested, leading to screening and admission areas being included within the ward. Patients wanted control and empowerment, like personal lockers, open access to various areas and private spaces. Six patients and a caregiver in a focus group suggested inpatient programmes. Peer Support Specialists (PSS) (caregivers and persons in recovery) were regularly involved in drafting process recommendations. Rapid experimentation of new processes was encouraged. EPIP ward became operational from 1 Feb 2016. The first dedicated to early psychosis, this 20-bedder mixed gender ward boasts features like open pantry and night lounge. Specially designed programmes and activities meaningfully engage, and promote and support recovery. Volunteers run activities while PSS facilitate support groups and instill recovery. EPIP has successfully involved patients and caregivers as advisors, process owners and collaborators in enhancing inpatient care.
